A Phase 3 trial turned the walls of a surgical cavity into the radiation treatment.

A multicenter randomized Phase 3 trial found that delivering radiation immediately from collagen tiles placed in the cavity during brain-metastasis surgery sharply reduced recurrence at the surgical site.

WHAT HAPPENED

Researchers randomized patients with newly diagnosed brain metastases requiring surgery to immediate cesium-131 tile-based radiation or standard postoperative stereotactic radiation. At one year, surgical-site recurrence was 1.3% in the tile group and 15.4% in the standard-treatment group. Median overall survival was also longer in the tile group, though that survival result should be interpreted within the trial population and follow-up.

WHY THIS IS HOPESLOP

The treatment is almost aggressively practical: remove the tumor, line the empty space with tiny collagen tiles, and let the radiation start before the incision is closed. A hard problem was answered by turning the cavity itself into part of the treatment.
